Advance Directives: Putting Your Health Care Wishes in Writing

October 1, 2026 · Quinebaug Valley Senior Center staff

An advance directive is a written plan for your health care if you become unable to speak for yourself. It gives your family and doctors clear guidance and takes a hard decision off their shoulders.

The main parts

  • Health care representative — the person you choose to make medical decisions for you if you can't.
  • Living will — your wishes about life-support and end-of-life treatment.
  • You can also name who should handle other matters, and state wishes about organ donation.

Things to know

  • You don't need a lawyer to complete Connecticut's forms, but they must be signed in front of witnesses. We can help you with the requirements.
  • You can change your mind and update your directive at any time.
  • Give copies to your representative, your doctor, and your family. Keep one at home where it's easy to find.
  • A free File of Life magnet keeps your medical information handy for first responders: (800) 814-1788.
